Went back to pdic office yesterday afternoon and spent one hour talking to the AVP for claims processing, Atty. Elaine.  More bad news, the personal appearance of the minor, if in the Philippines, is required for both filing of claims and collection of payout checks.  If outside of the country, then personal appearance in the embassy/consulate is required for authentication of birth certificate.   The minor if possible should have two valid ids- a school ID is acceptable while the other one would be a passport (what other id could a child have), aside from the birth certificate.   Bring all the originals together with photocopies- they will return the originals.   Atty Elaine admitted that they have changed the rules covering the legacy bank claims process because of so many problems ex. fictitious names, CTD exchanged for pre-need plans, etc.  That is why SPAs will only be allowed for those living outside of the country (aside from medical incapacity) and these will have to be authenticated in the phil consulates of the country.  Actually if you think about it, this limitation/restriction of the scope of the SPA can be challenged in any court of law because SPAs have always been sufficient to authorize a nominee to represent his or her principal for almost anything whether based abroad or living in the Phils.

I fully agree with you tacamonch.  An SPA notarized in the Philippines for Filipno residents MUST be enough, otherwise they dont give respect to our lawyers and our legal system!!!!! But i know this procedure of PDIC is only to give HARDSHIPS and EXCUSES for them to DELAY payments to us!!!!! Have MERCY on PDIC!!!!! DONT MAKE IT HARDER TO THE HAPLESS DEPOSITORS!!!!!!! ITS OUR MONEY!!!

onli in da pilipins! one government issued ID is not enough? two government issued IDs are required? imagine the cost of securing one government issued ID like a passport, driver's license, or postal IDs.

and will require a minor to be absent from the school just to show up for filing claims and collection of payout checks? if this is the case, the PDIC should work even on Saturdays and Sundays and schedule it for this purpose in order for the kids not to be absent from school because they changed the rule to suit their wild whims and caprices.

this early, they treat the minors like "criminals". and they release the depositor's insurance claims in a turtle pace scheme. what gives? this is the most terrible specie in the whole world. you already fell down, they will continue to attack, bite and suck you until you collapse.

          ^that's a good idea para inde maaksaya oras mo,ang CONS? mahuhuli lang tayo kumuha ng claim forms.I also have accnts in their Laguna and Batangas branch so i will ask my sister to go directly to PDIC head office instead para isang lakaran na lang.

since malapit ako sa rbop, i will claim accounts in ortigas pero all others with accounts in cebu, hintayin ko na i-claim sa pdic makati.

Is anyone else aware of this

My g/f told me that at the last meeting she attended, PDIC said they would be issuing the SPA forms that are to be completed.  I was going to do it myself here this Monday at the Philippine consulate in San Francisco.  I already have the forms completed and just need them signed at the consulate.  I don't want to have to pay for those, only to be declared void by the PDIC for whatever reason they make up on the day.  My g/f leaves the Philippines on the 25th to join me here in the States so I doubt I would get them back to her in time.

Anyone use their own POA forms rather than those of the PDIC?

The SPA forms distributed by PDIC has a different format, it is only applicable when both parties are in the same location so that both parties can sign them at the notary at the same time.
In cases when one is abroad, then that is not applicable.
I had my SPA notarized in Phil Consulate in New York. Thanks for that.  I was hoping this would be the case.  I'll go ahead and have the forms signed here and keep my fingers crossed they don't move the goalposts again come the day
